Mike Cavataio's tip-in put the Crusaders (10-21) up 63-62 with 56 seconds to play.
Mintz then hit a jumper on Lafayette's next possession, and after Holy Cross missed a field goal, made two free throws with 6 seconds left for the final margin.
The Leopards, who improved to 6-0 in conference tournament games at home, will play Friday in the championship game.
Mower went 5 for 10 from 3-point range and Jeff Kari added 10 for third-seeded Lafayette.
R.J. Evans led the seventh-seeded Crusaders with 21 points.
