Jon Jay strikes out in 4-2 count after umps, Cardinals lose track of count

The Rays beat the Cardinals by the score of 3-0 on Wednesday night (box score), and, with two outs in the seventh inning, Jon Jay struck out after everyone lost track of the count. It was effectively a 4-2 count and he should have walked earlier in the at-bat.

Here's video of the entire at-bat. You can see he took what was called ball three in a full count and continued the at-bat:

Manager Mike Matheny took responsibility for the mistake after the game and rightfully so. He told Derrick Goold of the St. Louis Post-Dispatch that it's his job to catch that in a "big moment."

The count is reviewable under the new instant replay system, so Matheny could have asked the umpires to check things out after calling what should have been ball four ball three. That's a pretty big mistake, especially in a close game.

Did Jay lose track of count too? Seems like it. Either that or he never bothered to say anything, which would be worse.
