Blue Jays manager John Schneider said Wednesday that Berrios has been diagnosed with a stress fracture in his right elbow and won't be available for Opening Day, Keegan Matheson of MLB.com reports.
Schneider noted that Berrios is pain free, and the hope is that the right-hander will resume throwing soon with the intention of pitching through the injury. With Shane Bieber (forearm) opening the season on the injured list and Trey Yesavage being brought along slowly to monitor his workload, Berrios could still have a significant role this season after finishing the 2025 campaign in the bullpen, but first he'll need to show he can be effective despite the elbow issue.

The injury was revealed on an MRI during the insurance process for the World Baseball Classic, for which Berrios was denied a chance to participate. The right-hander dealt with elbow inflammation down the stretch last year and also pitched through a biceps issue, but the current inflammation is apparently unrelated. Berrios said he's not experiencing any pain, and he'll continue to throw over the next few days as he continues to be evaluated.

The 31-year-old right-hander is looking to bounce back from a tough 2025 season, and Berrios was impressive Saturday against a New York lineup that featured Aaron Judge, Jazz Chisholm and other Opening Day starters. Berrios picked up velocity as the outing progressed, and his fastest pitch of the day registered at 94.5 mph on a four-seamer to top prospect Spencer Jones in the third inning -- a very good sign after he averaged just 93.0 mph with the pitch last season over 166.0 regular-season innings, the lowest mark of his career. Berrios admitted that he was already feeling less than 100 percent health-wise during spring training last year, but he didn't find out until September following an MRI that he had a biceps tendon issue. The righty wound up making 30 starts for the seventh consecutive non-COVID season, but he had a 5.15 ERA after the All-Star break and was demoted to the bullpen late in the year. Berrios endured a tough 2025, finishing the regular season on the IL due to a sore elbow and then getting left off the postseason roster. The veteran right-hander has made 30-plus starts in five straight seasons, with last year's 166.0 innings representing his lowest total for a full campaign since 2017 with Minnesota, but Toronto's offseason moves to bring in Dylan Cease and Cody Ponce seem to have Berrios ticketed for the Opening Day bullpen. He still has three years and $67 million left on his contract as well, which will make him tough to trade given his mediocre 4.17 ERA, 1.30 WHIP and 11.8 percent K%-BB% in 2025. With Shane Bieber (forearm) and Trey Yesavage both having potential workload concerns, however, Berrios could still end up getting plenty of starts in 2026 while filling a swingman role.

Berrios came away with a no-decision in Tuesday's 12-9 win over the Reds, surrendering six runs (two earned) on five hits and two walks over two innings. He struck out two.
The veteran right-hander hadn't taken the mound since Aug. 23, but the extended rest didn't seem to help Berrios, who needed 66 pitches (35 strikes) to record just six outs. The blow to his ERA was cushioned by an Isiah Kiner-Falefa throwing error in the second inning, but it's still the seventh time in 11 starts since the beginning of July that Berrios has been tagged for at least four runs, a stretch in which he carries a 5.47 ERA, 1.46 WHIP, 2.4 HR/9 and 42:14 K:BB over 52.2 innings. He tentatively lines up to make his next outing at home early next week against the Astros.

Berrios will have gone more than a week between starts, as he most recently took the hill in Miami this past Saturday, when he struck out eight batters and allowed two earned runs over 6.1 innings. The veteran right-hander doesn't seem to be dealing with an injury coming out of the start, but he'll be pushed to the back of the pitching schedule as the Blue Jays realign their rotation ahead of an important series this weekend against the NL-leading Brewers and next weekend against the Yankees, who sit four games behind Toronto in the AL East. The Blue Jays are now tentatively slated to have their top three members of the rotation -- Shane Bieber, Kevin Gausman and Max Scherzer -- make one start apiece in both of those series.
