Milano played in the Bills' last nine games consecutively before resting with most of the defensive starters in the team's Week 18 victory over the Jets. The linebacker appears to have acquired an illness, but it will likely not stop him from playing in Sunday's playoff game against the Jaguars. The 31-year-old will have two more opportunities to elevate his practice participation and prove he is over the illness in time for Sunday's contest.

Bills' Matt Milano: Limited by injury again in 2025
Milano recorded 67 tackles (36 solo) including 3.5 sacks and a forced fumble in 12 regular-season games with Buffalo in 2025.
While Milano was one of the Bills' top linebackers when he was available, an early-season pectoral injury and late-season load management limited him to just 12 games in 2025. He has now appeared in just 21 regular-season games in the past three years combined. He'll turn 32 in July, and it's unlikely he'll ever again match the highest snap shares he took up earlier on in his career. With that said, he's currently set to become a free agent this offseason and could be a steady contributor for a team that sees value in a veteran presence at linebacker in 2026.

Milano logged seven tackles (four solo), including 0.5 sacks, during the Bills' 13-12 loss to the Eagles on Sunday.
Milano and Greg Rousseau combined for a two-yard sack of Jalen Hurts late in the third quarter. Head coach Sean McDermott said Monday that Milano is "week-to-week" due to a pectoral injury that he aggravated during the first half of Sunday's 23-20 loss to the Patriots, Joe Buscaglia of The Athletic reports.
A pectoral injury kept Milano sidelined in the two games prior to Week 5. He was given the green light to play against New England, when he played 32 snaps on defense and made an immediate impact with six tackles (three solo), but the veteran linebacker did not see the field for the entire second half after aggravating the injury. Milano is in jeopardy of being sidelined for the Bills' Week 6 clash against the Falcons on Monday, Oct. 13, so his practice participation over the coming week will be worth monitoring closely. Shaq Thompson would be in line to see a bigger role at linebacker if Milano were to miss time.
