The 28-year-old Spanish citizen joined the London club in 2014 and has scored 59 goals in 120 matches. He joined from Atletico, where he had 64 goals in 135 matches.
After a falling out with Chelsea manager Antonio Conte, Costa hasn't played for the Blues this season with Alvaro Morata brought in as starter and Michey Batshuayi firmly set as Morata's back up.
Costa's move is expected to be completed during the January transfer window. Sadly for him, he couldn't get a move done during the summer with Atletico's current transfer ban. Had he been able to move in the summer, he could have faced Chelsea twice in this season's Champions League group stage as the two are in together in Group C.
