Packers' Zach Tom: Exits Saturday with head injury

Tom is being evaluated for a concussion and is questionable to return to Saturday's divisional-round contest against San Francisco, Bridget Condon of NFL Network reports.

Tom departed in the second half, and his return is uncertain. Yosh Nijman has entered at right tackle in Tom's stead.
